Pregnancy Waiver Liability waiver for all pregnant clients

Minor Waiver Parent liability waiver for all clients 17 and younger. Note: Parents must stay in the room during service of clients under the age of 13.

Media Release Waiver Release waiver to allow video and still recording of services. Any content gathered during the service will be used for promotional purposes on God's Healing Hands website and social media pages.
